“Health is a state of complete physical, mental and social well being not merely a state of absence of disease” – that is according to WHO.
I think you attain perfect health the moment there is a balance in these three dimensions. Generally, a person will have poor health the moment any of these three dimensions of health is doing badly.
Good health is a state in which one is able to function well physically(your body and organs), mentally(brain function), socially(how you interact with others), and spiritually(your values/beliefs) in order to express the full range of one’s unique potentialities within the environment in which one is living.
For good health you need : plenty of sleep, be physically active, avoid stress, drink plenty of fluids, and eat nutritious food.
Poor health may be due to personal(factors within you) or things in your environment(environmental).
Some of the main causes of poor health include: chronic stress, poor nutrition/a bad diet, poor sleep and lack of movement/adequate exercise.

There are many factors or reasons that may make someone to have a poor health. Some of these factors are natural, others are as a result of poor or risky behavior or habits a person does, and others may be social related factors.
Examples of natural factors may for example include someone having a poor or compromised immunity may is sometimes as a result of poor nutrition, being infected by certain illness or due to other genetic factors. Also, think of some other natural factors like climate change which may impact body processes such as direct exposure to sun rays.
Poor or risky behavior may include use of alcohol and drugs, having risky sexual behavior that may result into disease or unwanted pregnancies, lack of physical activity, tobacco smoking and others.
Social factors may include poverty which directly or indirectly results to lack of basic needs like nutritious food, proper housing and thereby leading to poor health. Also other social factors may include poor or lack of quality interaction and relationships that may result to poor mental health.
Overall, a number of factors are responsible for poor health and they their impact on health may depend on their intensity or multiplicity for example when someone is exposed to more than one or many of those factors
